Appereon Health Services - Behavioral Health

Located in Hyattsville, Maryland, the Appereon Health Services provides comprehensive behavioral health services throughout the Maryland area. We offer dynamic holistic rehabilitation services through our Community Integration, Outpatient, and Intensive Outpatient programs. Our programs and services are geared to meet the needs of the community. Our behavioral health solutions are built on years of commitment to quality, accessibility, and compassionate support. Our programs create a supportive environment for recovery, lasting wellness, and serve as a liaison and an advocate.  Our staff are dedicated to helping individuals overcome challenges and reach their full potential while remaining productive members in the community 

 Appereon Health Services offers Mental Health Services to adults and adolescents who have mental health challenges beyond substance abuse. We support the Increasing number of substance abusers also suffer from depression, anxiety, bipolar disorder, and other mental illnesses. Our Clients receive assessment and evaluation from licensed providers, medication management services, and individual and group therapy from licensed therapists in an outpatient setting.

Our staff are Substance Use Disorder (SUD) counselors who serve as a  liaison between individuals involved with government and community  partners (i.e. probation, the courts) who may be in need of SUD  treatment and other ancillary services. The target population includes  individuals who are involved in the criminal justice system, the  uninsured/underinsured, the homeless, and clients with co-occurring  substance use and mental health disorders. Services include: Outreach  and Engagement, Eligibility and Enrollment for public benefits, SUD  Education-Screening- Appointment Scheduling, Service Navigation and  Linkage, and Agency/ Community Education.

Appereon Health Services care management works together with care management staff provide services aimed at coordinating a patient's hospital care and discharge plan, as well as transitioning care back into the community and assisting with access to care, rides and other community resources. Our teams are made up of registered nurses, social workers, licensed practical nurses, pharmacists and health educators who work to ensure your care is coordinated.

The Appereon Health Services outpatient care management team works hand-in-hand with our primary care practices to help you meet your healthcare goals. We offer complete care with the expertise of not only your physician or advanced practice professional, but a registered nurse care manager, a social worker, and behavioral health therapist. Our goal is to work together to meet all of your healthcare needs.
